#3 is correct - . Loss of deep muscle "pennate" muscle fibers consistent with a deep contusion of the muscle

This is most likely a deep muscle contusion with irregular display of deep muscle structure over the area of injury

NOTES: A constant theme is that long axis (LAX) and short axis (SAX) views are required. The thought is that the blunt trauma forced the muscle up against the bone resulting in the contusion of the deep section of the muscle.  No clear disruptions of the muscle structure could be observed from these images.
